A Community-Centered Approach to Healthcare Access
Our Mission
The Clarkston Resource and Wellness hub is a project of Easterseals North Georgia, Inc. This project’s mission is to improve health outcomes by ensuring equitable access to necessary medical resources, reducing healthcare costs, and fostering a healthier, more resilient community. The success of this initiative could serve as a model for other communities facing similar challenges across the nation.
Services and Programs
The Hub offers a wide range of services, including culturally and linguistically responsive preventive and primary care, connections to specialty care, health education and access, transportation solutions, mental health education and services, children’s disability services, and cultural and linguistic support systems.
Additionally, The Hub utilizes a Community Health Worker (CHW) model, assisting families in identifying the social determinants that are impacting their health and well-being.
